Medical care comes first for urgent symptoms

After a neck injury, seek urgent medical assessment for tingling, weakness, trouble walking or sitting upright, an electric-shock sensation through your neck or back, or severe pain despite pain medicine. The NHS identifies these as urgent whiplash symptoms; they need more than a routine chiropractic appointment.

Call 911 or go to an emergency department after a head or body jolt if you have a worsening headache that won't go away, repeated vomiting, slurred speech, confusion, seizures, loss of consciousness or difficulty staying awake. See the CDC's concussion danger signs for more warning symptoms.

Your first step: a whiplash evaluation

Whiplash is a neck injury caused by a sudden movement of the head. Pain, stiffness, headaches or shoulder discomfort may begin hours after the injury. Symptoms alone can't tell you whether whiplash is the only problem.

We start by talking through the collision, your symptoms, health history and movement concerns. Tell us what hurts and what it interrupts, whether that's looking over your shoulder or getting comfortable at night. Bring existing scan reports and a medication list, if relevant, and wear clothing that lets you move comfortably.

The examination helps guide recommendations. Imaging depends on the findings and concern for injuries such as a fracture; it isn't automatic. A medical assessment or referral may be appropriate before treatment. This evaluation-first approach is consistent with SIRA's guidance for acute whiplash.

Exercises and help with everyday movement

Targeted exercises and education can give you ways to participate in care between visits. Ask which movements suit your symptoms and how to adapt activities that strain your neck. SIRA's whiplash guidance recommends staying active and appropriate neck exercises, including movement and strengthening exercises. The right starting point depends on your assessment.

Soft-tissue work and chiropractic adjustments

We may recommend hands-on work on muscles and other soft tissues alongside exercises or adjustments. Evidence for manual therapy in acute whiplash is limited, so it should be monitored and continued only with measurable benefit. Tell us if a technique makes symptoms worse or isn't helping.

Spinal manipulation uses a controlled thrust to a spinal joint. NCCIH's review of spinal manipulation describes possible benefits for some neck pain, with limitations in the research. That doesn't establish that an adjustment is right for every whiplash injury.

Manipulation can cause temporary soreness, stiffness or headache. Rare serious complications, including neurological problems, have been reported. Neck manipulation has been associated with artery tears that can lead to stroke; whether manipulation causes these tears remains disputed. Discuss the potential benefits, risks and alternatives before deciding. Share your medical conditions and medicines, and ask about options that don't involve a neck thrust.

Follow-up based on how you're doing

We monitor your response and discuss changes to care as your needs change. At follow-up, tell us what's easier, what's still difficult and any new symptoms. Turning your head, sleeping and sitting through work are useful examples to discuss.

If symptoms persist or worsen, you need reassessment. Depending on the findings, the next step may be a change in treatment or care from a medical clinician, physical therapist or pain specialist. Recovery varies, so we'll discuss visit frequency after your evaluation and revisit the plan as your needs change.

Yes. Symptoms can take several hours to develop. Tell the clinician when they began and how they've changed, even if you felt well just after the collision.
